Licensed Practical Nurse (LPN) (Healthcare) Hospice of Orange & Sullivan Counties is a nonprofit provider of compassionate end-of-life care across the Hudson Valley. We deliver comfort, dignity, and support to patients and families in their homes, hospitals, nursing facilities, and at our Kaplan Family Hospice Residence. Our interdisciplinary team focuses on physical, emotional, and spiritual care, helping patients and families navigate lifes most challenging moments. What We Are Looking For: Job Title: Licensed Practical Nurse - Days Schedule: Three (3), 12-hour shifts per week, 7:00 am - 7:30 pm. This includes an every other weekend (Saturday & Sunday) commitment. About the Role We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Kaplan Family Hospice Residence (KFHR) Clinical Care Team to care for patients within our 11-bed residence located at 1 Sunrise Lane, Newburgh, NY 12550. This role provides patient care under the guidance of an RN, following the individualized Plan of Care. The LPN will work closely with the hospice interdisciplinary team to ensure high-quality, patient-centered care that supports patients and families through every stage of hospice care. Essential Job Functions/Responsibilities: Provide direct patient care in accordance with the State Nurse Practice Act and established Plan of Care under RN supervision Accurately document care, patient condition, and pain/symptom management Communicate patient status changes promptly to the Registered Nurse Deliver effective pain and symptom management and evaluate patient response Educate patients and families on medications, treatments, diet, and self-care techniques Collaborate with the interdisciplinary hospice team to ensure coordinated care Adhere to infection control standards and all hospice policies and procedures #J-18808-Ljbffr HOSPICE OF ORANGE & SULLIVAN COUNTIES INC. in
