Clinical Social Worker Entry

Overview The Clinical Social Worker, Entry is a non-licensed Social Worker. The Entry level CSW conducts psycho-social assessments and interventions with patients/families and serves as a liaison with multi-disciplinary care team and external community agencies to implement interventions, while receiving instruction and guidance from higher level Clinical Social Workers. Duties & Responsibilities POPULATION SPECIFIC CARE Neonate – Infancy – >30 days to 1 year Toddlers – >1 year to 3 years Pre-Schoolers – >3 years to 5 years School age – >5 years to 13 years Adolescent – >13 years to 18 years Adult – >18 years to 65 years Essential Functions Develops, implements, and monitors service/treatment interventions that provide a variety of services to meet patient/family needs. May participate in department wide committees. Develops a therapeutic relationship with the patients and families to assist with the social, financial, and psychological problems associated with hospitalization and/or outpatient treatment. Develops and maintains relationships with peers within and outside of the department as well as community resources. Conducts initial needs assessment to gather information on the patient/family’s medical, social, and financial history. Serves as a liaison with multi-disciplinary care teams and external community agencies to identify, implement, and monitor treatment goals. Facilitates staffing, care conferences, etc. to coordinate care between the treatment team. Compiles and prepares case records, reports, and documents that comply with state and federal standards in providing case notes, treatment plans, and evaluations. May provide clinical supervision and support to clinical social work interns. Provides emotional support to multidisciplinary team members in the context of patient care. Participate in on call and coverage of other CSW team members. Scope & Level Guidelines: Trains in the field and assists higher level personnel to acquire learning through practical experience. Complexity: Performs routine and/or basic tasks common to the field. Procedures, methods, and techniques to be used are well established with options to be considered well defined. Tools, work aids, and materials to be used are specified. Decision Making: Decisions or recommendations on non-standardized situations are limited to relating organizational policies to specific cases. Brings non-routine issues to supervisor. Problems that are not covered by guidelines or that are without precedent are taken up with the supervisor. Communications: Contacts with team members, clients or the public where factual information relative to the organization or its functions are received, relayed, or a service rendered according to established procedures or instructions. Supervision Received: Under supervision, the employee receives assignments and is expected to carry them through to completion. Work is reviewed for adherence to instructions, accuracy, completeness, and conformance to standard practice or precedent. Frequency of supervision is determined by licensure status. Minimum Qualifications Degrees Master's Area of Study Social Work Licenses & Certifications Licensed Social Worker (LSW) in the state of Colorado (CO) or candidacy within 30 days of the start date if the new hire is a new graduate. Salary Information Pay is dependent on applicant's relevant experience. Annual Salary Range (Based on 40 hours worked per week): $68,066.41 to $102,099.61 Benefits Information Here, you matter.
