Availability

Start: ASAP upon credentialing
Schedule: Monday-Friday, 8:00 AM-4:30 PM
No call
No weekends
No after-hours
No telehealth
Ongoing outpatient coverage
Availability to provide requested coverage dates required

License Requirements

Active Illinois Medical License or IMLC
Active Illinois DEA
Active Illinois Controlled Substance Registration (CSR)

Required Certifications & Qualifications

Board Certified or Board Eligible in Internal Medicine or Surgery
Wound Care & Hyperbaric Medicine training and experience required
ACLS (AHA)
BLS (AHA)
PALS (AHA)

Submission Requirements

Board Certified or Board Eligible in Internal Medicine or Surgery - Required
Wound Care & Hyperbaric Medicine training and experience - Required
Active Illinois Medical License - Required
Active Illinois DEA/CSR - Required
Availability for requested coverage - Required
Two references per Stratum process - Required
Clean, easy-to-credential candidates - Highly Preferred

Practice Setting

Outpatient Wound Care & Hyperbaric Medicine
100% Adult & Geriatric patient population
12-20 outpatient wound care visits per day
1-4 Hyperbaric Oxygen (HBO) treatments per day
8 exam rooms
2 Hyperbaric chambers

Required Procedures

Wound evaluations and treatment planning
Compression therapy
Wound VAC management
Hyperbaric Oxygen (HBO) treatment oversight
Debridements
Cast applications
Advanced tissue applications
Incision and Drainage (I&D)
Callous paring
Chemical cautery

Optional Procedures

Punch biopsies

Support Staff

Experienced Wound Care RNs
1 Advanced Practice Provider in clinic Tuesday-Friday
