Job Description
Registered Nurse - Case Manager

Schedule

Day 5x8-Hour (08:00 - 16:30) with a variable 5-day workweek. Department operations occur 7 days per week, and days off may vary and are not guaranteed to be back-to-back, although every effort will be made to schedule consecutive days off when possible.

Assignment Details

Start date: ASAP. This position requires floating to a partner facility.

Requirements

• 2 years of acute medical Care Manager experience performing Care Coordination.
• California state license required; pending license accepted, but must be licensed upon start date.
• No more than 7 days of requested time off allowed.
• Travel experience is required; first-timers will not be accepted.

Responsibilities

• Coordinate patient care and ensure appropriate treatment plans are implemented.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
• Monitor patient progress and make necessary adjustments to care plans.
• Educate patients and their families on health management and resources available.
• Document patient information effectively to maintain compliance with facility policies.
