This position provides coverage throughout North San Diego County, including Vista, Oceanside, Escondido, San Marcos, and surrounding communities.

What You'll Do

• Provide direct patient home care visits, observes, performs and reports preventative, treatment and rehabilitative nursing procedures while implementing the plan of care
• Observe and report on the home environment and make recommendation that will enhance patient care, promote safety and prevent adverse effects
• Work with patient, family and health care team members to coordinate services, promote quality of care and positive patient care outcomes
• Take verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriately
• Inform Health Care Providers of changes in patient's condition in accordance with providers orders and documents communications accordingly
• Provide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventions
• Provide instructions to patient/CG's and staff in a timely manner

Qualifications

• Graduation from an accredited nursing program, current state license as a Licensed Vocational/Practical Nurse, and CPR-certified
• At least one year of clinical practical nursing experience. *Home Health experience preferred
• Must have a vehicle, a valid current driver's license, and automobile insurance
• Ability to make home visits and provide skilled nursing assessments and care in the home
