What Youll Do

• Provide skilled physical therapy services under the direction of the supervising Physical Therapist
• Implement individualized treatment plans designed to improve mobility, strength, balance, endurance, and functional independence
• Educate patients and caregivers on exercises, mobility techniques, and safety strategies
• Communicate patient progress and changes in condition with the supervising Physical Therapist and interdisciplinary care team
• Complete accurate and timely documentation while maintaining Medicare and agency standards

Qualifications

• Current Florida Physical Therapist Assistant (PTA) license
• Valid drivers license and reliable transportation
• Excellent communication and organizational skills
• Home health experience preferred, but not required
• Experience with WellSky or another home health EMR is a plus
