Physical Therapist - Travel Contract

Travel Physical Therapist — Home Health — Brooksville, FL

Provide skilled physical therapy services to home health patients in Hernando and Citrus County areas. Perform evaluations, start-of-care OASIS assessments, recertifications, and ongoing skilled treatment plans. Document using agency-required systems and coordinate care with interdisciplinary team members. Maintain professional licensure and compliance with state regulations while delivering patient-centered home visits.

Minimum Requirements

• Master‘s degree in Physical Therapy required; Doctoral or Bachelor‘s acceptable if meeting state licensure requirements.
• Active Florida Physical Therapy license required to start the assignment.
• Home health experience with OASIS assessments required.
• Experience with HomeCare HomeBase (HCHB) preferred.
• BLS certification from AHA or ARC may be required.
• Availability to complete SOC, evaluations, and recertifications as part of clinical responsibilities.

Preferred Qualifications

• Prior travel or per diem home health experience.
• Experience covering multiple counties or large geographic territories.

Assignment Details

• Shift hours: 08:00–16:00 (weekday schedule).
• Workdays: Monday through Friday schedule as provided.
• Weekend requirement: coverage for 2 Saturdays.
• Coverage area includes Citrus and Hernando County.
• Mileage reimbursed between visits from first patient to last patient.
