Registered Dietitian

Responsibilities Assess nutrition status and implement appropriate interventions to ensure participants’ needs are met. Review nutritional needs prior to enrollment into the program. Complete initial, semi‑annual, and annual assessments. Conduct comprehensive assessments that may include evaluation of: Diet recall Current diet Meal patterns Assistance with meal preparation and grocery shopping Anthropometric analysis Weight monitoring Appetite changes Difficulty chewing and/or swallowing Food allergies and preferences Monitoring for food insecurity Cultural and religious preferences Past medical history Evaluation of laboratory indicators Need for nutritional supplements and/or frozen meals Education follow‑up Reviews and determines appropriate therapeutic diets Work and collaborate with participants, caregivers, and the Interdisciplinary Team to develop the participant’s plan of care. Evaluate participant motivation for positive outcomes. Implement individualized interventions and adjust goals of care as needed. Supervise participants in the Day Health Center to ensure meals served comply with RDN recommendations. Communicate changes in dietary needs to the Interdisciplinary Team. Collaborate with Day Health Center staff and other team members to identify participants with chewing/swallowing difficulties and refer for evaluation. Counsel and educate participants and caregivers to enhance health and well‑being. Organize and maintain updated lists of frozen meals and nutritional supplements; approve and revise all cycle menus. Coordinate care with outside healthcare professionals. Maintain frequent contact and follow‑up with participants at high nutritional risk. Identify clinical characteristics of malnutrition. Calculate nutritional needs and coordinate with the Home Infusion Company for participants receiving enteral support. Promote the Participant Bill of Rights and strive for high participant satisfaction. Qualifications Earned a Bachelor’s Degree in Nutrition. Completed a dietetic internship or equivalent practicum and is registered as a Dietitian (RDN). Have at least one year of experience with aging adults. Participate in annual, mandatory in‑service training and screening, including infection control, TB testing, safety training, and BLS training. Possess professional analytical skills to analyze and solve problems and develop viable intervention plans. Establish and maintain interpersonal and interdepartmental relationships. Apply principles of adult learning in planning and delivering educational activities; conduct one‑to‑one teaching for participants, family members, and staff. Participate in and facilitate quality improvement projects based on data analysis. Ensure compliance with departmental and external standards by attending professional meetings and reviewing national standards of practice. Meet a standardized set of competencies before working independently. Be medically cleared for communicable diseases and have all immunizations up‑to‑date before engaging in direct participant contact. Working Conditions Exposure to communicable disease due to patient environment. Driving required within PACE of Southeast catchment area; may encounter extreme temperatures.
