Assignment Duration

This is a 13-week contract position.

Schedule

AM shifts with a schedule of Monday through Friday, requiring 40 hours per week.

Job Description

We are seeking a dedicated Home Health Physical Therapist, Occupational Therapist, or Speech Language Pathologist. The ideal candidate will provide essential therapy services to patients in various settings, focusing on evaluations, treatment planning, and delivery of care. Responsibilities will include managing a daily caseload of visits with a productivity standard of 28-32 points per week.

Responsibilities

• Conduct assessments and evaluations for patients in their homes.
• Develop and implement individualized treatment plans.
• Document all patient interactions promptly and accurately using Homecare Homebase or comparable systems.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Adjust treatment plans based on patient progress and outcomes.
• Participate in weekly case conferences.
• Provide patient education and support throughout the treatment process.

Required Experience / Certifications / Licensure

• Previous experience in Home Health and OASIS required.
• Documentation experience with Homecare Homebase is mandatory; candidates without HCHB experience may be considered at a reduced rate with training for the first two weeks.
• Active and valid professional licensure in Physical Therapy, Occupational Therapy, or Speech Language Pathology.
