Location Newport News, Virginia Overview The Speech‑Language Pathologist will follow appropriate practices to perform evaluations, timely re‑evaluations, and OASIS assessments; determine SLP diagnoses; develop a plan of care; and perform SLP procedures to aid in restoring/maintaining speech, language, motor, and swallowing abilities. The therapist is required to be flexible with daily scheduling, may need to work in various patient care settings, and may assist in coordination of Home Health Aides, participate in interdisciplinary case management, complete quality chart reviews, attend meetings as required by the facility/department, and engage in other initiatives to improve quality outcomes. What you will do • Performs evaluations, determines SLP diagnoses, develops plan of care, and performs speech therapy procedures to aid in restoring/maintaining speech, language, motor, and swallowing abilities. • Evaluates and assesses the patient timely, based on principles outlined by practice guidelines, evidence‑based guidelines and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc.). • Provides reassessments, including OASIS assessments at appropriate time points to determine the level of functioning. • Updates and revises the plan of care in consultation with the physician and interdisciplinary team members. • Provides therapeutic treatments and evaluates equipment needs as applicable. • Prepares clinical and progress notes that support skilled intervention and assess outcomes of care. • Plans for discharge of patient; builds trusting collaborative relationships with a multi‑disciplinary team to support patient care needs. • Actively participates in multi‑disciplinary team meetings, in‑services, and case conferences. • Carries out treatment plan effectively, modifies to maximize therapeutic benefit and meet discharge criteria and return to optimal level of care. • Utilizes testing results and develops an updated plan of care based on principles outlined in appropriate evidence‑based guidelines. • Coordinates with Home Health Aides in providing services, including completion of required supervisory visit documentation. • Participates in training and support of all team members, volunteers, and students. Qualifications Education: Masters Degree, Speech Language Pathology (required) Experience: 1 year Speech Language Pathology experience (preferred) Licenses and Certifications: Speech Language Pathologist (SLP) – The American Speech‑Language‑Hearing Association (ASHA) – Upon Hire (required) Speech Language Pathologist (SLP) – Virginia Department of Health Professions (VDHP) – Upon Hire (required) CPR/BLS Certification – American Heart Association/ American Red Cross/ American Safety and Health Institute (AHA/ARC) – within 30 days (required) Certificate of Clinical Competence in Speech Language Pathology (CCC‑SLP) – The American Speech‑Language‑Hearing Association (ASHA) – Upon Hire (required) Transportation: Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job; valid driver’s license required #J-18808-Ljbffr
