Role Summary

Key Responsibilities

Provide skilled physical therapy services to patients in their homes.

Perform comprehensive evaluations of patients' physical conditions and functional abilities.

Develop and implement individualized treatment plans tailored to patient goals and needs.

Educate patients and family members on therapeutic exercises, recovery strategies, and lifestyle modifications.

Monitor patient progress and adjust treatment plans to ensure optimal outcomes.

Collaborate with other healthcare professionals to support coordinated, comprehensive patient care.

Maintain appropriate clinical documentation in accordance with professional and regulatory standards.

Required Qualifications

Graduation from an accredited Physical Therapy program.

Active Physical Therapist licensure in the state of Connecticut.

Minimum of one (1) year of experience as a practicing Physical Therapist.

Strong interpersonal, communication, and patient-care skills.

Ability to work independently and manage time effectively.

Demonstrated commitment to compassionate, patient-centered care.

Preferred Qualifications

Prior experience providing physical therapy services in a home health setting.

Experience working with diverse patient populations in community-based care.
