Woodlake at Tolland is a short and long term care facility located in Tolland, CT. We are currently seeking a Full and Part Time RN Supervisor 3-11  shift to join our dedicated healthcare team. to join our team!Woodlake at Tolland is a top-rated skilled nursing facility known for its welcoming environment and commitment to quality care.

Key Responsibilities:

• Oversee and supervise nursing staff during the second shift, ensuring quality care is provided to residents.

• Perform assessments, administer medications, and manage treatments as ordered by physicians.

• Provide direct patient care and assist with any medical emergencies or urgent needs.

• Monitor and evaluate patient conditions, documenting changes in status as necessary.

• Ensure compliance with facility policies, regulations, and safety standards.

• Work collaboratively with other healthcare providers, including CNAs, physicians, and family members.

• Communicate effectively with the day shift team to ensure continuity of care and a smooth handover.

Qualifications:

• Current  Registered Nurse (RN)license in the state of Connecticut.

• Previous experience in a long-term care or skilled nursing facility is preferred.

• Strong leadership skills and the ability to manage and motivate nursing staff.

• Ability to work independently and make critical decisions in a timely manner.

• Compassionate, patient-centered approach with excellent communication skills.

• Ability to work the 3:00 PM - 11:00 AM shift.

• Must be detail-oriented and possess strong documentation and organizational skills.
