We are seeking a BHCM II / Promise Care Manager to join the DSAMH CMH PROMISE Program in Newark, Delaware.
The Promise Care Manager will work as part of an interdisciplinary behavioral health team serving individuals with severe and persistent mental illness (SPMI) , including individuals with co-occurring disorders .
The ideal candidate will have experience in behavioral health case management, care coordination, mental health services, treatment/recovery planning, assessments, and community-based support .
Key Responsibilities-
Perform behavioral health assessments and identify client needs.

Help develop and implement individualized recovery plans .

Determine appropriate levels of care and assist with placement.

Coordinate and monitor services to ensure care is delivered according to established standards.

Conduct community visits with clients/beneficiaries.
-
Maintain accurate and timely clinical documentation and charts .

Coordinate services with community-based providers and agencies.

Serve as a liaison between clients and community providers .

Collaborate with an interdisciplinary team of behavioral health professionals.

Support clients with accessing appropriate community resources and services.
-
Work with individuals experiencing serious mental illness and/or co-occurring disorders .

Bachelor's Degree required.

Valid Driver's License required.

Must be able to operate a vehicle for required community travel.

Behavioral health, mental health, case management, or care coordination experience is highly preferred.

This is an onsite, Monday Friday, first-shift position based at the New Castle Promise Assessment Center in Newark, Delaware . The position also requires community travel , so candidates must have a valid driver's license and be able to operate a vehicle.
