Monday–Friday, 8:00 AM – 5:00 PM | Weekend Rotation & On-Call Required

✅ Scheduled Hours: 40 Hours/Week (No Guaranteed Hours)

Minimum Requirements:

Active RN License
BLS Certification
Minimum 4 years of recent Case Management experience
Experience in inpatient and/or outpatient settings
Strong care coordination and discharge planning experience

Key Responsibilities:

Coordinate patient care across the continuum of care
Develop and manage individualized care plans
Facilitate discharge planning and community referrals
Collaborate with physicians, nurses, and interdisciplinary teams
Monitor patient progress and ensure quality outcomes
Support patients with complex medical and social needs
