RN Hospice Case Manager

Role: The RN Clinical Nurse is a proactive member of an interdisciplinary team responsible for delivering individualized, high-quality, and safe patient care. They practice according to established models, standards, and ethical guidelines, ensuring patient-centered care that respects cultural diversity. Key Responsibilities: Perform the nursing process, coordinate care, support performance improvement, and promote evidence-based practice and research. Foster teamwork through effective communication, documentation, and participation in shared governance. Serve as a preceptor or mentor to develop future nursing professionals. Plan and develop protocols to enhance patient outcomes and optimize resource use. Qualifications & Skills: Must have a BSN from an accredited program, current RN licensure (multi-state preferred), BLS certification, and minimum 10 years of direct patient care experience. Strong interpersonal, critical thinking, time management, and technological skills are essential. Ability to work in a fast-paced environment and respond decisively to emergencies is required.
