Family Case Manager

Family Case Manager Join us in our mission as a Family Case Manager at Catholic Charities Housing Office. We are committed to making a positive impact in our community. As the Family Case Manager, you will play a pivotal role in individuals' lives by: Completing timely and accurate documentation in the case record including referral intake packets, related information, referrals, correspondences, and entitlement documentation. Performing housing office assessments of each new tenant or head of household upon admission and on an annual basis thereafter. Developing and implementing Individual Service Plans (every six months), monitoring effectiveness, guiding family members toward positive outcomes, reporting outcomes, and making changes as needed. Maintaining case record notes documenting progress toward milestones and performance targets in the ISP. Conducting case conferences with the Director of Multi‑Family Housing, CCHO staff, DSS workers, and other treatment providers as needed. Cooperating in ongoing supervisory processes with DSS, including attendance at scheduled case management team meetings. Attending meetings and advocating on behalf of clients and the program, escorting clients to appointments as required. Salary: $20.00–$23.00 per hour. Schedule: Monday–Friday, 11:00 am–7:00 pm (37.5 hours per week). Qualifications Bachelor’s degree in a related field. 1–3 years of experience working with a diverse population.
