The Hamilton County Health & Social Services Division has a responsibility to respond quickly and efficiently when public health emergencies occur. All Hamilton County Health & Social Services Division employees are responsible for ensuring their contact information is current and they are accessible should a public health emergency arise. The Hamilton County Health & Social Services Division maintains a call down system to store contact information and mobilize staff in the event of an emergency. During a public health emergency, all staff are required to respond should a call to mobilize be made. During a public health emergency, work hours for any and all Hamilton County Health & Social Services Division employees are subject to modification to meet the needs of the community. Under general supervision is responsible for providing individual and group social counseling services, support, clinical case management, conduct comprehensive psychosocial assessments, develop treatment plans, and support patients in achieving their behavioral health and recovery goals. This may include facilitating Intensive Outpatient Program (IOP) groups in person three to four times per week.

With a focus on high-risk individuals, provides trauma-informed individual and group mental health services and referrals to individuals experiencing homelessness, housing insecurity, substance use disorders, severe mental illness, and co-occurring behavioral health conditions. Provides critical client support through assessment, crisis intervention, care coordination, community outreach, and linkage to housing, healthcare, and social service resources. Works collaboratively with multidisciplinary teams and community partners to support stabilization, recovery, and continuity of care.
Responsible for providing crisis intervention, behavioral health screenings, needs assessments, safety planning, individual therapy, group therapy, psychoeducation, and referrals for clients experiencing homelessness or housing instability. Will develop and update individual treatment plans.
Develops rapport and therapeutic relationships with clients utilizing trauma-informed, culturally responsive, and harm reduction approaches.
Develops, implements, monitors, and updates individualized treatment and service plans based on client needs, goals, and level of functioning.
Facilitates evidence-based group interventions, psychoeducational programming, life skills groups, relapse prevention groups, and wellness oriented activities.
Conducts community outreach and engagement activities in shelters, encampments, transitional housing programs, mobile outreach settings, clinics, and other community locations.
Assists clients with linkage and referrals to housing resources, medical care, psychiatric services, substance use treatment, social services, employment resources, benefits, and community supports.
Maintains, updates, and manages clinical documentation, case records, treatment plans, referrals, and other required paperwork in compliance with agency, state, and federal requirements.
Collaborates with multidisciplinary teams, healthcare providers, outreach staff, and community agencies to coordinate care and support continuity of services.

ADDITIONAL REQUIREMENTS :
Prior to operating a County-owned vehicle, all employees are required to complete the HC Defensive Driving course and/or an Emergency Vehicle Operations Course (EVOC) provided at the department level.
This position is deemed essential during inclement weather situations and must report to or remain at work, even when administrative closings are announced, as determined by the Department Head.
PREFERRED QUALIFICATIONS:
A Master's of Social Work Certificate (CMSW) or License of Clinical Social Work (LCSW) from the State of Tennessee is preferred.
