Fishkill Home Care is seeking a caring Social Worker to provide quality House visits to our clients.  We are looking for individuals who desire a worthwhile career that has purpose and makes a meaningful difference in someone’s life. The Social Worker will be responsible for the following .

Ø  This is a per diem position;

Ø  Up to 10 patients per week will be scheduled - we are paying $40-55 per visit (plus mileage).

Responsibilities:

• Conducts home visits, intake assessments, scheduling, evaluations, case management, crisis intervention and follow- up services.

• Participates in the service delivery components, follow-up, outreach, entitlements and benefits assistance, advocacy, etc.

• Assists with assessment and matching of clients with extensive follow-up

• Prepares and organizes case files

• Answer program related telephone inquiries and provides information and referral services

• Makes follow up phone calls clients after a match is made

• Assists with record keeping and in insuring timely preparation of all required reports.

• Assists with follow up to physicians

• Other tasks as assigned

Requirements:

• Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in directing the overall operation of the facility's delivery of services.

• Long term care/ sub-acute experience required

• Performs resident evaluations as required by state regulations.

• Support state regulations for admission and retention of residents.

• Assists residents and families with the transition process.

• Orients residents and families to the facility.

• Assists and interacts with residents and families to access services.      

• Knowledge of discharges planning and assistance that address the identified needs of adult populations.

• Communicates with other members of the management team to ensure coordination of services to residents.

• Documents case management notes in residents' files as necessary.

• May perform other duties as assigned.

• Knowledge of MDS and discharge planning are essential.

Qualifications:

• BSW or Master’s Degree in a related field required.

• Experience in the field of social work providing services to an adult population as required by state regulation if applicable.

• Two to five years experience in social work providing services to an adult population is preferred.

• Basic computer knowledge, particularly in Microsoft Word and Excel.

• Certified/Licensed as required by state regulations if applicable.

• Valid driver's license.
