Job Summary
We are looking for individuals to join our client’s team as a Licensed Master Social Worker to provide patient support, discharge planning, care coordination, and referrals to appropriate post-acute and community resources.
Job Description
- Attend interdisciplinary rounds and collaborate with physicians, nurses, and care teams on patient discharge planning
- Complete psychosocial assessments, care plans, patient education, and EMR documentation within required timeframes
- Meet with assigned patients regularly and provide discharge planning support and aftercare referrals
- Coordinate referrals for home care, skilled nursing facilities, rehabilitation, and community services
- Utilize Allscripts, Careport, and Parachute systems for referrals, orders, and care coordination
- Ensure timely patient notification of discharge and compliance with departmental policies
Skills Required
- Minimum 1 year of LMSW experience in a hospital, inpatient, acute care, or medical/surgical setting
- Strong discharge planning and care coordination experience, including referrals to home care, SNFs, and community resources
- Strong communicator who can engage patients and families, coordinate discharge plans, and collaborate effectively with clinical teams
- Organized, detail-oriented, compassionate, and able to manage a high-volume caseload while maintaining accurate documentation
- Proactive problem-solver with knowledge of community resources and post-discharge care planning
Preferred Skills:
- Experience/Knowledge of EPIC Use
- Experience of inpatient medicine or surgery service/unit in a hospital
- Experience using Allscripts, Careport, and/or Parachute applications
- Knowledge of community resources, post-acute care services, and social determinants of health
- Experience working within interdisciplinary healthcare teams in an inpatient setting
Education/Training/Certifications
- Master’s degree in social work
- Active LMSW license issued by NYSED (Primary Source Verified)

- Schedule notes: 9:00 AM-5:00 PM
Pay Rate
- $52–$61 per hour, based on experience, skills, and market factors
