The Home Services Social Worker provides psychosocial assessments and interventions for patients and families, addressing emotional, social, and environmental needs related to illness and life situations. Responsibilities include developing bereavement care plans, interpreting cultural and faith traditions, educating families, and facilitating communication and decision-making. The role involves documenting assessments, implementing care plans to enhance family support, maximize benefits, and respect patient dignity, especially at end-of-life. The social worker collaborates with interdisciplinary teams, responds to imminent death indicators, and educates on advanced directives. Key qualifications include a Master's Degree in Social Work , licensure as an LSW or LCSW in Illinois, and at least one year of hospice experience .
