Outpatient Physical Therapist

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. The therapist serves as an educator for patients, families, students, and coworkers while demonstrating behaviors consistent with the organization's Mission Statement and Core Values.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Document medical records in a clear, concise, accurate, and timely manner in compliance with relevant guidelines and clinic policies.
• Provide care under signed, valid physician orders and within insurance approval criteria.
• Participate in student programs by supervising and mentoring students, developing learning objectives, and completing evaluations.
• Actively pursue professional development through continuing education and involvement in professional activities.
• Adhere to discipline-specific state practice acts and maintain licensure in good standing.
• Engage in performance improvement initiatives and provide feedback for maximizing performance outcomes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• CPR certification.
• Active, applicable state license for physical therapy.
• Previous experience with our agency is highly desired.

Schedule

This is a 13-week contract position with a 5x8.0 schedule.
