As a Social Worker/Care Companion, you will play a vital role in ensuring the emotional well-being of older adults in long-term care settings.

About the Role

As a Social Worker/Care Companion, you will visit designated facilities in accordance with your supervisor's schedule to conduct assessments, implement short-term therapy and supportive interventions, and collaborate with psychiatric professionals to provide comprehensive care.

Key Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

Qualifications:

• Associate, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree
• Experience: 1 year
