Physical Therapist - Travel Contract

Minimum Requirements

Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
At least 1 year home health experience
Experience with HCHB
Physical Therapist lisenced in the state of TX

Assignment Details

Shift Hours: 8:00 AM - 4:30 PM
Shift Days: Monday - Friday
On-call requirement: 1 weekend per month
Coverage area: Dallas service area
Productivity expectation: 30 points weekly
Top responsibilities include OASIS and subsequent visits
