The BHUCC Nurse
Duties:
Assists with crisis interventions and medical emergencies, obtains vital signs each shift or more frequently as indicated.
Follows and understands CIWA protocol when BAL is above 0.000 and administers correct detox medication per protocol.
Communicate with other healthcare professionals during recovery partnership and transition phases, and liaison with other agencies as indicated.
LPN is responsible for triaging referrals, completing LPN intake/assessments on all clients admitted during their shift, obtaining urine drug screens, BAL and pregnancy tests on admissions, obtaining admission standing orders, dispensing routine and PLPN medications, taking orders from prescriber and updating MAR, completing electronic and paper charting in a timely manner and participating in treatment team meetings when required.
LPN is required to contact correct leadership in regard to denials and medical clearance.
Typical Working Conditions/Environment
This position requires the LPN to provide direct medical care to clients, during 12 hour shifts, on a 23 hour Behavioral Health Urgent Care Unit.
This position is required to provide nursing covering on a 12 hour shift despite any weather conditions or holidays that arise.
Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, and going down on knees, running, and walking.
Daily Staff Duties:
Performs the initial admission nursing assessment and completes required admission paperwork including nursing assessment, nursing note and nursing admission orders.
Update medication tabs.
Update allergy lists.
Complete paper MAR.
Obtain vital signs, urine drug screen, BAL and pregnancy tests upon admission.
Follows all established protocols and guidelines for admission prior to admitting the client to the unit.
Makes the determination of the appropriate admission protocols needed for each client (ex.-agitation protocol, diabetic protocol, HTN protocol, CIWA protocol).
*CIWA protocol must be completed Q4H regardless of client being asleep.
Obtains admission orders from the on-call prescriber in a timely manner. Night shift will utilize the on-call PM prescriber for crisis services when medical advice or additional medication is needed.
Completes charting for each client during every shift prior to the end of shift.
Completes transition paperwork and discusses medication and aftercare instructions with client and/or family. Provides copy of paperwork for clients.
Counts narcotics and documents at every shift change. Notify Team Lead immediately of any discrepancies.
Obtains and charts vital signs on every shift.
Monitor for significant changes in client's condition/behavior every shift and make referrals to obtain new orders as needed.
Administer scheduled, routine, and PLPN medication in a timely manner.
Obtains blood sugars as ordered.
Triages potential referrals with Mobile Crisis and Walk in Center.
Calls appropriate leadership for denials. Medical denials will be handled by Crisis Services Nursing Manager.
** All other denials must be discussed with Program Coordinator or Program Director before client is denied.
Complete C-SSRS on each client
Attend to guest and/or client needs during medical emergencies (ie: seizures, falls, etc)
Coordinate with Emergency Departments and EMS
Provide simple wound care when indicated.
Oversee client meals and snacks.
Complete nursing discharge note.
Update Admission/Discharge log each time a client is admitted or transitioned from unit.
Maintain safe and clean working environment by complying with procedures, rules, and regulations (ie: infection control procedures/policy, medication administration and storage procedures, and controlled substance regulations).
Maintain client confidence and protect operations by keeping information confidential.
Team Building:
Participates in orientation and training of new staff.
Participate in shift reports.
Coordinate transitions with other staff.
Maintains a positive and professional working relationship with all staff members and supervisors.
Communicates and approaches conflict in a problem-solving manner with staff and supervisors.
Accepts additional assignments and/or changes in assignments to meet the needs of Crisis Services. This includes training and covering Crisis Stabilization Unit shifts when deemed appropriate by Crisis Services Nursing Manager and Program Coordinator
Assists Mobile Crisis and Walk in Center when necessary to obtain vital signs on clients being referred to a higher level of care.

Schedule:
Schedule requires 12 hours shifts, rotating weekends, and scheduled holidays.
Regular attendance is an essential job function.
Mandatory to remain awake and alert during shift.
Must be punctual and maintain a good attendance record.

Equipment/Technology : Basic computer skills are required for email, timekeeping, and documentation in the electronic medical record.
QUALIFICATIONS - BHUCC Nurse
Education:
LPN with current Tennessee license and experience in psychiatric and alcohol and drug treatment setting preferred.
CPR certification or willingness to become certified is required.
Experience / Knowledge:
Should have sound knowledge of psychiatric and detox medications, diagnoses, treatment modalities and interventions.
Experience in working with special populations including individuals with Severe and Persistent Mental Illness and Co-Occurring Disorders preferred.
Experience working in a crisis setting preferred.
Physical:
Minimal exposure to biological hazards.
Hearing of normal/soft tones and close eye work.
Frequent expose to unpleasant odors.
Schedule requires 12 hour shifts.
Lift up to 50 lbs.
Pushing/pulling up to 250 lbs.
Valid driver's license.
Frequent sitting, standing, walking, bending, stooping, and reaching.
CPR and First Aid certification required (training provided).
Required to be certified in and adequately implement verbal de-escalation techniques.
Applicants should be able to exercise sound judgement under pressure.
Clinical staff may be required to get an F endorsement to transport clients as necessary.
