Nurse Case Manager

FlexStaff is hiring a part‑time temporary Nurse Case Manager to join our client's team. Schedule will vary each week (3 days per week) - will be coordinated between site and employee. Hours will be 9 a‑5 p. Pay $80.00 per hour Location Fishkill, NY Length of Assignment 13 weeks Caseload The caseload is 25‑30 people. The number of houses varies depending on the number of residents. We have homes with 4‑14 people. They must go to each house they cover a minimum of once per week. MaryJane was told that if another nurse was out, she may have to cover a house and the location of our homes. Position Summary Coordinate the delivery of health care services for persons receiving services as assigned in a residence and or program unit. Make nursing assessments and diagnoses. Work in conjunction with other members of the Interdisciplinary Team (IDT) to achieve positive health outcomes for persons receiving services in compliance with OPWDD and applicable regulations. Requirements Maintain current NYS Registered Nurse license. Graduation from an accredited nursing program. Bachelor’s Degree preferred. Prior nursing experience and/or experience working with persons with developmental disabilities preferred. Excellent oral and written communication skills. Competence in computer skills – Microsoft Office, Electronic medical record experience preferred. Ability to travel to and access multiple locations throughout agency locations. Flexible work schedule with hours and/or days – as approved by the Director of Nursing. Perform all job responsibilities in full compliance with all applicable laws, rules, regulations, policies, and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal False Claims Act, whistleblower protection, and mandatory reporting. Complete corporate compliance training at least annually. Fully cooperate in all corporate compliance investigations and reviews.
