Licensed Practical Nurse (LPN) - Developmental Disabilities Residential Services

The Licensed Practical Nurse (LPN) provides direct nursing care, health monitoring, and medical oversight to individuals with developmental and intellectual disabilities living in a group home, day services or independent living environments. This role supports the health, safety, and well‑being of residents through person‑centered care, medication management, coordination with healthcare providers, and strong collaboration with direct support professionals (DSPs). Key Responsibilities Health Care & Nursing Support Conduct routine health assessments and monitor residents’ physical and emotional health. Provide basic nursing care, including wound care, vital signs, and chronic condition monitoring. Administer medications and treatments in accordance with state regulations and agency policy. Ensure accurate Medication Administration Records (MARs) and medication counts. Respond to medical emergencies and communicate effectively with management. Care Planning & Documentation Follow and implement physician orders and resident‑specific health plans. Document nursing notes, health incidents, changes in condition, and follow‑up care promptly and accurately. Participate in interdisciplinary team meetings and help update individual service plans (ISPs). Maintain compliance with state developmental disability regulations and agency policies. Advocacy, Support & Training Educate residents on health, hygiene, and wellness skills when appropriate. Support residents in making informed choices about their healthcare needs. Train and support DSP, Foster Care and agency staff as directed on health‑related tasks as needed, such as medication administration, seizure protocols, dietary plans, infection control, and use of adaptive equipment. Medical Coordination Schedule and coordinate medical, dental, and therapy appointments. Accompany residents to appointments as needed and relay information to the care team. Communicate changes in health status to guardians, case managers, and providers according to policy. Safety & Compliance Maintain infection‑control standards and monitor for signs of illness. Ensure medical supplies, equipment, and emergency kits are stocked and functional. Follow all HIPAA, OSHA, and state licensing requirements for developmental disability services. Qualifications Current, active Licensed Practical Nurse (LPN) license in good standing. Valid CPR/First Aid certification (or willingness to obtain). Ability to pass criminal and abuse registry background checks. Valid driver’s license, acceptable MVR and current auto insurance. Strong communication skills and ability to work with individuals with diverse physical and cognitive abilities. Experience in developmental disabilities, behavioral health, group home, home health, or similar settings. Knowledge of person‑centered planning and trauma‑informed care. Experience with electronic health record (EHR) systems. Knowledge of Therap a plus. Work Environment Community‑based group home setting, day center and independent living environments. Requires lifting, transferring, or supporting individuals as needed. May include rotating shifts, on‑call duties, or weekend/holiday coverage depending on agency needs. #J-18808-Ljbffr
