We are seeking a positive, hardworking, and experienced Nurse Practitioner or Physician Assistant to join our team and provide excellent customer service to our patients! Sugar Lakes Family Practice is a comprehensive medical group, caring for patients of all ages. We focus on preventive medicine and health maintenance by providing wellness exams, routine and sports physicals, and patient education. We help our patients share in the responsibility of exercising good health practices for themselves and their families. We provide medical care for most conditions including, but not limited to, heart disease, diabetes, hypertension, and thyroid disease. We also perform biopsy and cryotherapy for skin lesions, repair lacerations and treat many orthopedic conditions.

Duties and Responsibilities include, but not limited to :

• Provider shall perform all reasonable duties, within the scope of practice of a nurse practitioner licensed in the state of Texas. Provider shall, without limitation, practice in accordance with the state's laws, rules and regulations.
• Provider shall practice as a member of an interdisciplinary health care team for all patients over the age of two (2) years old. As a member of this team, the Provider provides primary health care through direct care, consultation, collaboration, coordination and/or referral.
• Provider shall observe the office hours established from time to time by the Employer and shall perform direct care to the patients assigned to her by the Employer.
• Provider shall work a minimum of 40 hours per week
• Provider shall also complete all necessary documentation and charting, including but not limited to office visit notes, related to encounters with patients who have been assigned to her, and office notes shall be closed and locked within seventy-two (72) hours for billing and legal purposes.
• Provider shall keep and maintain (or causing to be kept and maintained) appropriate records relating to all services rendered by her under the Agreement.
• Provider shall prepare and attend to, in connection with such services, all reports, claims, correspondence necessary or appropriate in the circumstances, all of which records, reports, claims and correspondence shall be the property of the Employer.
• Provider shall perform all things reasonably desirable and necessary to maintain and improve her professional skills.
• The Employer shall direct, control and supervise the duties and work of Provider; provided, however, the Employer shall not impose employment duties or constraints of any kind which would require Provider to infringe the ethics applicable to the scope of practice of a nurse practitioner licensed in the state of Texas or violate any ordinance or law.

Required Qualifications:

Certification/Skills

• Current Texas Licensure as a Registered Nurse
• Current Texas Licensure as an APRN – NP
• PA-C credential, licensed and credentialed by the Texas Board of Physician Assistants
• Prescriptive authority privileges through the State of Texas
• Drug Enforcement Administration (DEA) registration within 6 months of hire
• Basic Life Support (BLS) certified or specialty certification as required by department

Minimum Education:

• Master’s in Nursing as NP or Master’s degree in Nursing with completion of a post-masters Nurse Practitioner Certification Program
• Bachelor’s degree
• Graduate from accredited Physician Assistant program by NCCPA

Preferred Qualifications:

• One (1) year of work experience as a Nurse Practitioner or Physician Assistant in a family practice or pediatric clinic setting
• One (1) year of work experience with documentation in eClinicalWorks
• Knowledge of meaningful use quality measures
• Bilingual
