You will work collaboratively with an interdisciplinary team to deliver high-quality, patient-centered care.### ### **Key Responsibilities*** Perform **patient evaluations** and develop individualized treatment plans* Provide therapeutic interventions to improve **strength, balance, mobility, and function*** Monitor patient progress and adjust treatment plans as needed* Maintain accurate and timely clinical documentation* Communicate with physicians, families, and care team members* Participate in **care planning, discharge planning, and rehab meetings*** Supervise **PTAs, therapy aides, and students*** Provide **in-service training and education** for staff### ### **Qualifications*** Degree in **Physical Therapy** (Bachelor’s required; Master’s or Doctorate preferred)* Active, unencumbered **Physical Therapist license in South Carolina (required)*** Experience in a **skilled nursing or rehab setting preferred*** Familiarity with **PointClickCare (PCC) and Casamba preferred*** Strong clinical, leadership, and communication skills* Commitment to continuing education and professional development### ### **Why Join Reedy River Post Acute?**At Reedy River, you’re part of a team that values your expertise and invests in your success.
