Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to conduct annual in-home health assessments for Medicare Advantage and other health plan members in King County, WA. This non-prescribing, field-based role focuses on preventive care, patient education, care gap closure, and collaboration with primary care providers.
