Competitive pay: $50.00-$55.00/hour DOE Sign-On Bonus Available Relocation Assistance Available Full-Time and Part-Time opportunities available Flexible scheduling options Medical, Dental, Vision, and Life Insurance for eligible employees 401(k) with employer match Paid Time Off and Holiday Pay Continuing education and professional development opportunities Supportive rehabilitation, nursing, and leadership teams Modern therapy equipment and treatment resources Collaborative interdisciplinary environment Career advancement opportunities Beautiful Lake County location with a strong sense of community Physical Therapist Assistant (PTA) Responsibilities Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist Implement individualized treatment plans designed to improve strength, mobility, balance, endurance, and functional independence Assist patients with therapeutic exercises, transfer training, gait training, and mobility activities Monitor resident performance, response to treatment, and progress toward goals Communicate patient progress and treatment outcomes to supervising Physical Therapist Educate residents and families on exercise programs, mobility techniques, and safety strategies Collaborate with nursing staff, physicians, therapists, and interdisciplinary care teams Maintain accurate and timely documentation in accordance with regulatory and facility requirements Participate in care planning meetings and discharge planning activities Ensure a safe and positive therapy experience for all residents Qualifications Current California Physical Therapist Assistant (PTA) License required Graduate of an accredited Physical Therapist Assistant program Strong communication, organizational, and interpersonal skills Compassionate, patient-centered approach to care Ability to work effectively within an interdisciplinary healthcare team Experience in Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care settings preferred Knowledge of rehabilitation principles, therapeutic exercise, and mobility training preferred New graduates are encouraged to apply About Lakeport Post Acute At Lakeport Post Acute, we believe outstanding patient outcomes begin with exceptional caregivers.
