Physical Therapist - Travel Contract

Home Health Physical Therapist

Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services in a home health setting in Pollocksville, NC. The clinician will evaluate, plan, implement, and document patient-centered care to restore or maintain patient function. Collaborate with interdisciplinary teams, instruct patients and caregivers on safe mobility and equipment use, and adapt interventions to home environments. Expect physical demands that include lifting, bending, prolonged standing, and local travel to patient homes.

Minimum Requirements

• Current State Physical Therapist license for North Carolina required.
• Bachelor of Science, Master of Science, or Doctoral degree in Physical Therapy from an APTA-approved program required.
• Proof of current CPR certification required.
• Current driver's license, reliable transportation, and vehicle insurance required.
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting required.
• Ability to perform patient assessments, prepare and revise Plans of Care, document progress notes, and implement therapeutic interventions.
• Ability to perform physically demanding tasks including heavy lifting, prolonged standing and walking, and travel to patient homes.
• Ability to read, write, and speak fluent English.
• Must meet mandatory continuing education requirements and maintain professional competency.
• Ability to work a flexible schedule and travel locally.

Preferred Qualifications

• Previous home health experience preferred.
• Experience with durable medical equipment and ordering appropriate rehabilitative equipment preferred.

Impacting Patient Care

Provide evidence-based physical therapy interventions to patients across the lifespan. Perform clinical assessments to determine needs and appropriateness of care. Develop, document, and update Plans of Care in consultation with physicians and interdisciplinary teams. Educate patients and families on transfer techniques, assistive devices, and home safety.
