Job Title: Registered Nurse – Adult Care / Home Care

Location: Bronx, NY, United States
Job Type: Contract
Shift: Day Shift | Monday–Friday | 9:00 AM – 5:00 PM
Duration: Approximately 13 Weeks
Hours: 37.5–40 Hours per Week
Weekend Requirement: Every Other Weekend

Job Overview

"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse to provide adult home care services throughout the Bronx. The RN will deliver individualized patient care for post-acute and medical-surgical patients in home settings while ensuring quality care, patient safety, and continuity of treatment.

Key Responsibilities

Provide direct nursing care to patients in home care settings
Assess, plan, and implement individualized patient care plans
Administer medications and monitor patient responses
Perform wound care, IV therapy, and post-operative nursing care
Monitor vital signs and report changes in patient condition
Educate patients and families regarding treatment and recovery plans
Maintain accurate documentation in electronic medical records
Coordinate care and discharge planning with interdisciplinary teams
Follow infection control and patient safety protocols
Travel locally within the Bronx Borough to patient homes

Required Qualifications

Active New York State RN License required
Minimum 2 years of recent Med-Surg experience in acute or post-acute care
AHA BLS Certification required
Associate Degree in Nursing (minimum requirement)
Strong skills in patient assessment, medication administration, IV therapy, and wound care
Ability to work independently in home care environments

Preferred Qualifications

Home care or visiting nurse experience preferred
EPIC EMR experience preferred
Knowledge of OASIS documentation preferred
ACLS Certification preferred
