NOW HIRING a Physical Therapist for per diem OR part-time opportunities in the home care setting. You will be paid for each completed patient visit (with documentation). **FT opportunity for the right individual!

WORK INDEPENDENTLY  *  1-to-1 IN-HOME PATIENT CARE  *  NO OASIS (we have RNs for that!)

RESPONSIBILITIES: 

• Perform home visits for patients in a home care setting
• Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition
• Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition

QUALIFICATIONS:

• Current license as a Physical Therapist in Connecticut
• Valid driver’s license
• Reliable transportation and willing to travel in New London and/or Windham and/or Middlesex Counties
• Strong communication and interpersonal skills
• Home health experience including Medicare preferred
