Registered Nurse (RN)

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our dynamic clinical care team. In this role, you will provide direct patient care, manage triage services, and support a collaborative team of 12 providers, 2 LVNs, 6 MAs, and additional clinical staff. Your day-to-day responsibilities will include assessing and implementing care plans for patients with both routine and complex needs, responding to patient concerns both in-person and by phone, and ensuring seamless coordination among care team members. Ideal candidates will bring experience in chronic disease management, triage, care coordination, and a commitment to quality improvement initiatives. Proficiency with Epic or other EMR systems, as well as experience mentoring or supervising LVNs, MAs, or nursing students, will be highly valued. This position offers the opportunity to contribute to upcoming initiatives aimed at improving clinical workflows and team efficiency, such as enhanced inventory management and empowering staff to work at the top of their licenses. Functions as a member of collaborative interdisciplinary team providing comprehensive care to patients and families with complex healthcare needs that may be both predictable and unpredictable. Aims to provide care that is safe, effective, patient-centered, timely, efficient and equitable.

Performs responsibilities in a manner that promotes quality patient care and customer service/satisfaction.

Adheres to broad scope of practice as established by Nurse Practice Act.

Ensures the efficient operation of the clinic or unit by proper scheduling and planning for optimum use of staff and resources, maintenance of stock and supplies, and efficient and safe use of equipment.

Uses systematic approach to provide nursing care to patients and families/populations by performing focused and comprehensive assessments through subjective and evidence based objective data.

Prioritizes, synthesizes and documents assessments and nursing plan of care using critical thinking and judgment.

Implements plan of care within scope of practice through assignment and monitoring of other nurses and delegation and monitoring of unlicensed staff.

Initiates standing orders for prescription refills and labs as well as standing delegation orders where appropriate.

Communicates effectively with collaborative team while escalating critical results and issues in an appropriate and timely manner.

Exercises judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care where indicated.

Performs onsite and telephone triage using best practice guidelines and documentation to ensure patients receive the right level of care in the right setting at the right time by the right caregiver using the right equipment and supplies.

Acts as a patient and healthcare advocate by maintaining a safe environment and promoting quality healthcare and services.

Respects and maintains privacy and confidentiality per HIPAA guidelines.

Adheres to standards of practice for professional nursing.

Acts as a clinical and leadership resource for team members.

Develops and implements teaching plans concerning maintenance, promotion and restoration of health. Evaluates patients' responses and outcomes to education and therapeutic interventions.

Performs other duties as assigned.

Uses typical office and clinic equipment.

Registered Nurse (RN) State Licensure or Nurse Licensure Compact (NLC) by Texas Board of Nursing (BON) required

Basic Life Support (BLS) by American Heart Association (AHA) required

Advanced Cardiovascular Life Support (ACLS) by American Heart Association (AHA) preferred

Bachelor's Degree in nursing from an accredited nursing school with one year nursing experience required Associate's Degree in nursing from an accredited nursing school with three years nursing experience required Nursing Diploma (3 year program) and two years nursing experience required

Years of experience varies based on education level required

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
