Certified Nurse Assistant

As a Certified Nurse Assistant with Touchstone Health's Hospice team in San Antonio, you'll provide hands-on, compassionate care to hospice patients in the comfort of their own homes. You'll support activities of daily living, monitor vital signs, and offer the kind of steady, personal presence that helps patients maintain dignity and comfort at one of life's most meaningful stages. This role is a natural fit for someone who wants to build real relationships with the patients and families they care for, not just check tasks off a list.

Key Responsibilities

• Provide personal care services in accordance with each patient's plan of care and under the supervision of a licensed nurse
• Assist patients with bathing, grooming, dressing, mobility, toileting, and feeding
• Measure and record vital signs including temperature, pulse, respiration, and blood pressure
• Observe and report changes in patient condition or care needs promptly to supervising staff
• Assist with ambulation, transfers, and positioning to promote comfort and prevent complications
• Maintain accurate documentation of care provided in the EMR
• Provide emotional support and companionship to patients and families
• Participate in in-service training and continuing education

What We're Looking For

• Current certification as a Nurse Aide in the state of Texas
• Completion of a state-approved Nurse Aide Training program
• High school diploma or equivalent
• At least one year of experience preferred; hospice experience a plus
• Strong communication and interpersonal skills with patients, families, and staff
• Reliable transportation, valid driver's license, and auto insurance for home visits
• Someone who brings patience, warmth, and steadiness into every home they enter
