Payout is based on employment in the exact role at the time of payout. Internal team members are not eligible. Schedule Requirements Full-time position Rotating weekends required Must work 1 summer and 1 winter holiday Travel required throughout Near North City and North Suburbs How You Will Make a Difference In this role, you will support patient care delivery as part of the home health team, working under the direction of the Clinical Manager. You will: Assist with patient care activities for a designated group of patients Support the delivery of care aligned with Advocate Health at Home standards Help ensure patients receive high-quality, cost-effective care Promote and support patient and family rights Contribute to maintaining and improving overall quality outcomes Required Qualifications Active license in the State of Illinois Minimum of 2 years of medical-surgical (med/surg) experience Comfortable using computer-based scheduling and documentation systems Strong written and verbal communication skills Effective interpersonal and teamwork skills Compensation & Benefits Hourly Pay Range: $24.35 – $36.55 Actual compensation is based on experience, skills, and internal equity.
