Overview
Physical Therapist – Outpatient, Lebanon, Oregon, United States. Travel - Temporary Assignment. Start date 09/28/2026 for 13 weeks. Weekly pay range: $2,368 – $2,468 per week. Guaranteed hours: 40 per week. Shift: 4x10-hour days (0700–1700). The position is in an outpatient rehabilitation setting.

- Shift: 4x10-hour days, weekdays only (no weekends)

- Work setting: outpatient rehabilitation department affiliated with a health system Responsibilities
- Conduct initial and ongoing PT evaluations for outpatient patients and develop individualized treatment plans.
- Deliver evidence-based interventions for sports and orthopedic rehab, pediatric rehab, neurological and stroke rehab, balance and dizziness rehab, mobility and extremity care.
- Provide gait training, therapeutic exercises, manual therapy and modalities within scope; monitor progress and adjust plans as needed.
- Educate patients and families on home exercise programs, safety, fall prevention, and functional strengthening; document outcomes and progress.
- Collaborate with the care team, document in the medical record, and communicate patient status across care settings.
- Maintain safety, infection control, and patient handling best practices; travel to multiple points of care as assigned. Requirements
- Active, unencumbered Oregon Physical Therapy license
- Healthcare Provider Level BLS certification
- Ability to travel to multiple points of care
- Familiarity with outpatient rehabilitation principles and workflows
