\n\n Job Summary: \n
\nAPN: Practices professional nursing by the performance of both independent nursing and delegated medical functions in accordance with accepted practice standards.
\n PA: Practices medicine with collaborating physician(s) in accordance with accepted practice standards. \n Utilizes specialized knowledge, judgment, and skills involving the application of biological, physical, social, and behavioral science principles. Functions include patient advocacy and the initiation and performance of medical care to evaluate, diagnose, and treat human disease, pain, injury, physical or mental conditions, health maintenance and promotion, supportive and restorative care. \n\n
Essential Responsibilities: \n

- DATA COLLECTION \n
- Obtains a pertinent medical history, performs an appropriate physical examination, and reviews relevant laboratory or radiologic (or other) studies to assess the patients medical, physical, psychological, and relevant socio-economic status. \n
- Will have full access to patient medical records and other information pertinent to those patients under his/her care. \n
- ASSESSMENT \n
- Formulates an accurate and appropriate diagnosis based on elicited data and includes differential diagnoses as appropriate. \n
- TREATMENT PLANNING \n
- Develops and implements treatment plans that are aligned with the practice preferences of the Collaborating Physician(s) including orders for medication and/or other treatment modalities, the need for additional testing, consultation to other providers, and follow-up provisions. \n
- Provides care with attention to safety, efficiency, and cost effectiveness. \n
- Plans are appropriately prioritized and adapted to the patient and circumstance. \n
- PROCEDURAL CONSIDERATIONS \n
- Performs procedures within the limits of their expertise and in accordance with the State Regulations and Organizational Policies. \n
- PATIENT EDUCATION AND COUNSELING \n
- Educates and counsels patients and their families regarding their treatment plan, including disease prevention and health promotion. \n
- Promotes the availability of culturally sensitive health and resource information that is evidence-based, patient centered, and enables informed choice. \n
- DOCUMENTATION \n
- Documents all examination data, diagnostic impressions, appropriate codes, and other patient care activities in the medical record, including the name(s) of any consulted physicians or other providers. \n

- Observes the principles and techniques of Universal Precautions. \n
- Conducts telephonic and virtual encounters as appropriate. \n
- Manages assigned in-basket tasks. \n
- Takes necessary and appropriate actions in emergency situations. \n
- Performs other clinical/administrative duties as directed. \n

- Maintains a working knowledge of, and complies with relevant (Federal, State, and/or Organizational) laws, rules, and policies; including licensure and certification. \n
- Educates and promotes the roles of the APN/PA to patients and members of the health care team. \n
- Must adhere to established policies, procedures, and/or guidelines in place for administering care to designated patient population. \n

- Graduate Degree in Nursing \n
- Graduation from accredited nursing program approved by CO State board of Nursing. \n
- For PA: \n
- Bachelors Degree required \n
- Graduation from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) \n
\n License, Certification, Registration\n
\n- \n
- Registered Nurse License (Colorado) AND Nurse Practitioner National Certificate AND APN Prescriptive Authority License (Colorado) required at hire AND Advanced Practice Nurse Registration (Colorado) \n
OR
\n- \n
- Registered Nurse License (Colorado) AND Nurse Practitioner National Certificate AND APN Prescriptive Authority License (Colorado) within 24 months of hire AND Advanced Practice Nurse Registration (Colorado) AND APN Prescriptive Authority License - Provisional (Colorado) required at hire \n

Physician Assistant License (Colorado) AND Physician Assistant Certificate

Drug Enforcement Administration Registration within 3 months of hire

\n Additional Requirements: \n

- Must obtain and remain credentialed by the KP Colorado Credentialing by start date. \n
- Candidates will be required to participate in a clinical/behavioral and panel interview composed of Management, CPMG, and Local 7. \n
- Taxonomy code required at time of hire \n
- Additional Requirements for APN: \n
- Education and Certification must be congruent with population focus of the position. (Example: for positions in Pediatrics, the APN must be certified as either an FNP-BC or PNP-BC). \n
- Must obtain Full Prescriptive Authority within two years of hire date. \n
