Schedule

Day shift, 5x8 hours (08:00 - 16:30), Monday to Friday, with a weekend rotation of every 4th weekend.

Job Description

We are seeking a dedicated and compassionate Registered Nurse to join our Home Health team. The successful candidate will provide high-quality patient care in a home setting, ensuring the health and well-being of patients through comprehensive assessments and tailored nursing interventions.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient progress and communicate findings to the healthcare team.
• Manage complex patient care needs, including OASIS documentation.
• Educate patients and families on health management and disease prevention.
• Coordinate with interdisciplinary teams to ensure optimal patient outcomes.

Required Experience / Certifications / Licensure

• Minimum of 2 years of nursing experience required.
• Current Nursing License (ASN or BSN) with BLS certification.
• Experience with OASIS documentation is mandatory.
