Family Practice/Primary Care Nurse Practitioner

A community-focused healthcare organization in Gilroy, California, is seeking an experienced Nurse Practitioner to provide comprehensive care across women s health, family planning, school-based health care, and primary care.

Position Highlights:

• Provide comprehensive women s health, family planning, and primary care
• Support school-based health care services as appropriate
• Perform comprehensive health histories and physical examinations
• Diagnose, treat, and develop individualized care plans
• Prescribe medications within scope of practice
• Perform and interpret laboratory testing
• Provide case management and coordinate referrals
• Perform women s health procedures, including Pap smears and specimen collection
• Educate patients and communities on contraception, STDs, and other health concerns
• Maintain accurate electronic medical records and clinical documentation
• Provide guidance and in-service education to nursing and clinical support staff
• Participate in special projects, training, meetings, and community activities
• Collaborate with Clinical and Medical Directors while working independently within scope of practice
• Some positions may support multiple clinic locations , based on organizational needs

Qualifications:

• Current California Nurse Practitioner license
• National certification through the American Academy of Nurse Practitioners or another recognized national certifying body
• Completion of an accredited Nurse Practitioner program
• Minimum 2 years of clinical experience
• Strong knowledge of nursing, diagnostic, laboratory, and medication management practices
• Experience with women's health procedures and specimen collection
• Excellent clinical judgment, communication, and problem-solving skills
• Ability to work independently and maintain effective relationships with patients, staff, and community partners

• $74.88 $86.81 per hour
