Where you'll be working
Based in the welcoming rural town of Scone, this role offers the opportunity to make a meaningful difference across both inpatient and community settings. While Scone will be your primary base, you may occasionally be required to provide support at other sites across the district.
You will join a friendly and supportive Community Health team, where Allied Health professionals collaborate closely and provide in-reach services to local hospitals and Multi-Purpose Services. This is a diverse Occupational Therapy role providing services to adult inpatients and community clients, including those receiving Palliative Care.

- Conduct assessments, interventions, and discharge planning
- Ensure safe and effective transitions from hospital to home or community
Community Responsibilities
- Support clients to achieve optimal outcomes in home safety, community access, and connection to appropriate community supports.

You Will
- Manage your own caseload and prioritise tasks effectively
- Conduct inpatient assessments and plan safe, timely discharges
- Collaborate closely with other disciplines, particularly in the inpatient setting
- Have high-level verbal, written, and interpersonal communication skills, with the ability to adapt to different audiences
- Be able to work autonomously and collaboratively within a multidisciplinary team to manage complex situations

Additional Information
- * Based on a minimum 18-month commitment to the position or the agreed duration of your contract. All payments to part-time and contract employees will be made on a pro‑ratio basis in accordance with the terms of your contract, including any approved extensions. Please note: if your contract ends before the agreed completion date, you may be required to repay a portion of the incentive. - This position is full time; however, part time/job share arrangements may also be considered.
- To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa.
