Social Work Clinician

Social Work Clinician Under the direction of the Manager or designee, perform diversified duties both within and in support of social work functions. Under the supervision of an LICSW, provide case management services, including the application of clinical diagnostic, assessment, and treatment knowledge or skills. Identify and address psychosocial factors that may impede patients’ ability to optimize benefit from the health care delivery experience. Intervene as necessary to prevent over‑ and under‑utilization of health care resources. Actively participate in discharge planning to ensure timely patient discharges within the scope of position responsibilities. Position follows a hybrid schedule. Qualifications A Master’s Degree in Social Work from an accredited college or university. National third‑party certification, LCSW, required within 4 months of hire. A minimum of MSW required. Must be fully vaccinated against seasonal influenza and COVID‑19, or obtain an exemption by signing a statement.
