Under the direction of the registered nurse, the purpose of this position is to provide holistic care for patients and families served.

Qualifications

Required:

• Current CA Certified Nursing Assistant certification
• HeartSaver AED
• Kaweah Health approved crisis intervention training within 60 days of hire/transfer.

Preferred:

• High school diploma or equivalent
• Healthcare experience

Knowledge/Skills/Abilities:

• Able to exhibit a courteous and friendly manner to others
• Organizational and computer skills necessary to support the needs of the department

Job Responsibilities

Essential:

• Greets visitors and interdisciplinary team members with a smile and provides assistance as appropriate within the scope of their training.
• Collaborates and communicates with the nursing staff to maintain accurate records during the patient's hospitalization. Communicates effectively with different departments and people. Assists staff with communication within and between departments. Keeps charge nurse informed or admissions, discharges and transfers by frequent communication. Performs electronic updates to appropriate systems for admissions, discharges and transfers (i.e. patient disposition, assignment management application, throughput application). Accurately enters patient data into computer on all admissions, discharges, and transfers. Accurately and legibly maintains staffing assignment sheets.
• Utilizes appropriate resources for clarification of orders. Transcribes orders as directed by unit procedures or downtime processes. Follows all departmental downtime procedures. Able to perform all regular duties when the hospital computer system is not available.
• Prepares and maintains patient's charts. Reviews charts at the end of every shift to ensure adequate supply of forms are filed in the chart and that the chart is maintained in proper order. Ensures that after discharge the patient's chart is completely assembled for Medical Records. Prepares teh chart in proper order for patients transferred to another unit.Accurately and legibly maintains staffing, record of admissions and discharges, sick calls, floating, and docking per established unit process. Notifies Case Management (when on unit), when orders are written for discharge or discharge planning (i.e. placement, durable medical equipment, home health, acute transfers).Obtains Medical Records for physicians from other facilities.Arranges for a follow-up appointment with Primary Care Physicians (PCP). Refers to Case management if they do not have a PCP.Files all hard copy medical records appropriately in the patient chart, i.e. lab results, progress notes, operating room records, EKG strips, downtime order forms. Copies chart and all materials as directed and distributes to appropriate people.Is an active participant in achieving quality patient care.Refers complaints and problems to the Nurse Manager, Assistant Nurse Manager or Team Leader. Holds self and other team members accountable to maintain standards. Maintains confidentiality with all patient care issues.Answers patient or family questions or refers them to the appropriate personnel.Answers patient call light and directs requests to the appropriate person. Ensures the patients request is received by a person who can assist them timely. Answers phone within four rings.
• Keeps work area and nurses' station clean and well organized.
• Orients students, new unit staff, and student volunteers to the unit's clerical function. Mentors new hires on computer usage appropriate to their position/unit.
• Assists nursing staff in achieving quality patient care by transporting specimens and retrieving items needed for patient care. Is certified to pick up blood from the Lab.
• Performs data collection for audits and other performance improvement initiatives as delegated by the unit leader or charge nurse. Utilizes the electronic health record reports and dashboard (i.e. Clinical Leader Organizer) to monitor and collect information as assigned.Reads and complies with current hospital/patient care procedures and policies as applicable.
• Complies with current policies and procedures including safety and infection control. Must keep all required certifications current. Performs tasks safely.
• Assists the Nursing staff in performing tasks as delegated by the Registered Nurse.
• Provides for the toilet needs of patients by emptying bedpans and drainage container. Reports any variation in amount, color, consistency, and odor of output to the RN.
• Provides assistance with the admission, transfer, and discharge of patients.
• Orients patients to room (call light, storage of belongings, etc.).
• Assists patients with meals, snacks, and with water pitchers.
• Assists with activities of daily living: bath, ambulation oral care, etc.
• Collects and records intake and output, weight, height, and vital signs as delegated by the Registered Nurse. Collects respiratory rate by direct observation of the patient.
• Documents all care provided in the EMR as soon after the care occurs as possible.
• Collects, labels, and delivers specimens.
• Provides post-mortem care and assists in transporting the body to the morgue.
• Assists with emergency care by participating in crash cart checks, providing cardio-pulmonary resuscitation, and responding to Code Blue.
• Handles the personal property of the patient with great care.
• Personal appearance shall reflect a well-groomed, professional appearance as outlined in HR.107, Professional Appearance Guidelines. Watches with timing capabilities, i.e. seconds hand or stopwatch, are required uniform components.
• Maintains a clean, safe, and neat patient environment.

Addendum (essential for specific dept)

TRANSITIONAL CARE UNIT OR SUBACUTE:

When assigned to work as a Restorative Nurse Assistant: Assists residents to achieve their highest level of physical functioning in ADL activities such as mobility training, range of motion and splinting skills. Functions under the supervision of the Registered Nurse.

PEDIATRICS ALSO RESPONSIBLE FOR:

Performs Unit Secretary duties for the unit.

OBSTETRICS ALSO RESPONSIBLE FOR:

May stock IV fluids and Irrigation fluids in locked supply drawers on mother/baby unit.

CVICU AND CVICCU ALSO RESPONSIBLE FOR:

May handle syringes/needles for the purpose of restocking such items in appropriately secured areas.

May check and document medication storage temperatures in accordance with hospital policy. Reports any temperatures that are outside of the recommended storage range immediately to the Nurse Manager (or designee) for their follow-up.

May access the medication room for the purpose of accessing needed supplies and equipment. Maintains medication room security and fully understands and complies with ensuring proper security of medications in accordance with hospital policy. Reports any concerns immediately to the Nurse Manager (or designee).

CVICU ALSO RESPONSIBLE FOR:

May perform phlebotomy duties if certified.

Additional:

Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.

Performs other duties as assigned.

Pay Range: $20.38 -$30.57
