divh2Registered Nurse Supervisor/h2pThe Registered Nurse is responsible for the delivery of patient care services through coordination, implementation, and supervision of field staff. The Registered Nurse Supervisor ensures that field staff follows the Plan of Care and delivers care accordingly. The Registered Nurse participates in quality improvement activities within the agency, promoting overall compliance with State and Federal guidelines and professional standards of practice./ph3Position Qualifications:/h3pMust be a registered nurse with a public health, home health care, or hospital background/ppMust have at least 2 years clinical experience/ph3Physical Qualifications:/h3pMust be able to hear and speak in a manner understood by most persons./ppMust be able to travel to prospective patients place of residence/ppMust be able to stoop and bend; must be able to lift and transfer patients./ph3Duties and Responsibilities:/h3pDemonstrates leadership and provides direction to members of the patient care team through case management of patient home health episode of health care. Case management of patient services includes:/pulliInitial in home evaluations and/or assessment/liliEstablish the patients individual treatment and medical record baseline./liliCollaborate with physicians; other agency staff and contract services to coordinate and implement the Plan of Care/liliImplement the Plan of Care and the quality of care delivered to the patient/liliProvide in home supervision of field staff at least every 90 to 180 days for patient receiving aide services and document the visit in the medical record (EHR)/liliProvide supervision of field staff assigned to patient caseload, through directing the implementation and evaluating the effectiveness of the nursing care and patients plan of care/liliRe-evaluate the patient plan of care at least every 90 to 180 days and when there is a change in the patient condition, after a hospital stay, and a time of discharge revising the Plan of Care as needed to achieve patient goals./liliConduct new employee orientation and in-services as needed./liliDocuments direct patient services, coordination and collaboration with physician and other disciplines or provides in the care of the patient, ensuring document is complete, and complies with acceptable home health standards and Agency policy./liliDocumentation is timely and meets professional standards of practice, establishes compliance with state regulations and Medicaid conditions of participation./liliReviews caseload and charts documentation to ensure compliance and assesses patient progress and effectiveness of care delivery in meeting patient goals./liliEnsures that MD orders are faxed timely, follow with MD offices to ensure timely receipt/liliConducts employee physical as needed/liliCompletes employee annual evaluations/liliF/U calls to clients/families and physicians on all clinical matters/liliF/U calls to clients/families and referral sources to respond to complaints/liliCollaborating with leadership/management team and office staff to facilitate timely office, processing of paperwork and personnel information/liliActively participates in Agency quality improvement activities/liliPerforms other duties as assigned/li/ulpstrongBenefits:/strong/ppFlexible work schedule/ppMileage reimbursement/ppPTO/ppstrongSchedule:/strong/ppMonday - Friday (Schedule to be discussed during interview)/ppstrongPay:/strong/ppThe pay range is 40-45/hr based on experience/p/div
