In-Home Physician Per Diem - Floyd

Signify Health, part of CVS Health® is seeking a Per Diem Physician to provide In‑Home Health Evaluations. You will meet patients in the comfort of their own homes and deliver personalized, comprehensive health assessments. Job Highlights The visit lasts approximately 45 minutes and includes a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the health plan (e.g., diabetic eye exam, spirometry). Physicians are not required to prescribe medicine, order lab tests, or alter existing treatment regimens. Responsibilities Conduct in‑home evaluations, building trusted relationships with patients and fostering health improvements. Use an iPad and an intuitive clinical workflow for each visit. Provide Diagnostic & Preventive Services (e.g., spirometry, DEE) as indicated. Fulfill all requirements of the specified role and collaborate with Signify Health clinicians. Travel locally and within licensed states as needed; conduct virtual visits when necessary. Work Environment Your primary workplace is the private residence of health plan members, which may contain pets, secondhand smoke, or other airborne allergens. Candidates must be able to safely perform duties in such diverse environments. Compensation Hourly base rate: $100.00 – $100.00. Pay depends on experience, education, geography, and other factors. Additional incentives are available per completed Diagnostic and Preventive Services. Qualifications MD or DO, active and unrestricted license(s) in the coverage area(s). Multi‑state licenses preferred. Ability to cover primary locations in the Floyd region. Strong communication and patient‑care skills.
