In-Home RN Case Manager

LeadingAge Virginia

LeadingAge Virginia is seeking an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to maintain the participant in their home environment. The role requires an active RN license in the working state and at least 1 year of nursing experience; hospice or hospital settings are preferred, with EMR/EHR system experience and a valid driver’s license.
