# Case ManagerHUTCH - Hutchinson, KS 67502## OverviewPosition TypeFull TimeJob ShiftDayEducation Level2 Year DegreeTravel PercentageNegligibleCategoryNonprofit - Social Services## DescriptionRole: Coordinates communication, services, and all levels of support for assigned individuals.Major Duties and Responsibilities:* Participate in and provide information for annual eligibility assessment.* Coordinate and communicate with relevant Managed Care Organizations for all care plan meetings and updates.* Develop Person Centered Support Plans (PCSPs) while meeting KDADS timelines and expectations.* Coordinate services derived from the PCSP, with routine follow-up reviews of progress and outcomes.* Develop and oversee implementation of Positive Behavior Support Plans.* Communicate with individuals served and their families to assist in oversight and coordination of services.* Work with residential and day service program managers to address any concerns related to services and care.* Assist with mental health and medication administration meetings to ensure accuracy and implementation.* Attend all required meetings.* Complete required billing documentation daily.Expectations:* Prompt, consistent attendance is an essential function of this position.* Understand and support the mission statement of Disability Supports of enriching the lives of those we serve, as evidenced by an unfailing positive attitude in all actions on behalf of individuals served.* Timeliness in completion of assigned tasks.* Play a proactive and positive role in identifying and seeking solutions for issues surrounding individuals served, as well as their comprehensive services and support.## QualificationsKnowledge and Skills:* Experience: Two years prior experience in the field of human services/developmental disabilities preferred.* Education: Bachelors degree in human services related major.* Interpersonal Skills: Strong verbal, written and negotiation communication skills required. Ethical decision constructive, and proactive decision-making skills are required. Must consistently exhibit unfailing compassion for individuals with disabilities.* Other Skills: Basic mathematical and intermediate computer skills required. Experience with Microsoft Office Products preferred.* Physical/Environmental: Ability to lift a minimum of 35 pounds. Travel to appointments and conferences as needed. Moderate noise such as would be found in a business office with computers and printers, light traffic. #J-18808-Ljbffr
