Location: Aurora, CO Status: Full-time Schedule: Monday-Friday, 8:00 a - 4:30 p Annual Pay Range: $89,772 - $98,300 Supplemental Pay: Based on position, schedule and/or availability: Shift Diffs/Stipends Mileage and Expense Reimbursement: Sixty-two and a half cents per mile – one of the highest in the industry! Will work to maximize the participant’s health outcomes and participant experience by identifying participant needs, completing pre‑visit planning, supporting the clinic visit and providing closure to the clinic visits, including scheduling follow‑up appointments, testing, lab work, and participant instructions. Will respond to walk‑ins and/or triage calls. Responsible for thoroughly and appropriately completing clinic visits designated as nursing visits. Essential Duties Provide skilled nursing care and assessment for PACE participants in accordance with nursing standards and scope of practice. Conduct medication reconciliation, monitor medication compliance, and evaluate treatment effectiveness. Assess participant needs, triage concerns, and respond appropriately using approved protocols. Deliver participant and caregiver education tailored to individual care goals and identified needs. Ensure participant privacy, comfort, safety, and continuity of care across clinic, home, and community settings. Prepare participants for examinations and support efficient clinic flow. Participate in on‑call nursing rotation and respond to urgent participant care needs as required. Collaborate with the Interdisciplinary Team (IDT) to develop, communicate, and adjust participant care plans. Coordinate care transitions and communicate effectively with community providers, pharmacies, hospitals, and extended care facilities. Attend participant, family, and interdisciplinary care meetings as requested. Promote effective communication and teamwork across clinical, administrative, and community partners. Maintain accurate, timely, and complete documentation in the electronic medical record (EMR). Ensure compliance with organizational policies, professional standards, and all applicable local, state, and federal regulations. Maintain participant confidentiality in all communications, records, and interactions. Follow established procedures, protocols, infection control practices, and safety standards. Participate in quality improvement initiatives and support organizational performance goals. Maintain clinical competency and participate in required education and professional development activities. Utilize technology and information systems to support participant care, documentation, and operational efficiency. Manage time and organizational resources effectively to support high‑quality, cost‑effective care delivery. Support organizational flexibility by adapting to changing participant needs, schedules, and census demands. Establish therapeutic relationships with participants and families from diverse cultural and socioeconomic backgrounds. Demonstrate professionalism, strong interpersonal communication, and excellent customer service. Represent Colorado PACE positively in the community and support organizational growth initiatives. What We Are Going To Love About You Graduate of an accredited school of nursing; BSN preferred. One year of professional experience required, minimum one year experience working with the frail and elderly population, home care or geriatric experience. Valid RN License. Valid Colorado Driver’s License and proof of Auto Insurance in accordance with state statute. Physical Requirements Ability to lift/carry a minimum of 35 lbs.
