Position Summary

Sunbright Hospice Care is seeking a compassionate and dedicated Per Diem Hospice Chaplain to provide spiritual, emotional, and bereavement support to hospice patients and their families. The Chaplain serves as a member of the interdisciplinary team, helping patients find comfort, meaning, and peace while respecting diverse faith traditions, beliefs, and cultural backgrounds.

Essential Duties and Responsibilities

• Conduct spiritual assessments and provide ongoing spiritual care to hospice patients and families.
• Offer prayer, counseling, religious rituals, and emotional support based on patient preferences.
• Support patients and families through end-of-life transitions, grief, loss, and bereavement.
• Collaborate with physicians, nurses, social workers, and other interdisciplinary team members.
• Participate in interdisciplinary team (IDT) meetings as requested.
• Coordinate care with community clergy and faith leaders when appropriate.
• Document all patient visits and interventions accurately and timely in the electronic medical record.
• Maintain patient confidentiality and comply with all hospice regulations and agency policies.
• Provide culturally sensitive care that honors each patient's values, beliefs, and wishes.

QualificationsRequired

• Experience in chaplaincy, pastoral care, ministry, counseling, hospice, healthcare, or a related field.
• Strong communication, active listening, and interpersonal skills.
• Ability to provide compassionate support to patients and families from diverse backgrounds.
• Valid California Driver's License, reliable transportation, and current automobile insurance.
• Ability to travel throughout San Joaquin and Stanislaus Counties.

Preferred

• Bachelor's Degree in Theology, Divinity, Religious Studies, Pastoral Care, or related field.
• Clinical Pastoral Education (CPE) training.
• Previous hospice or palliative care experience.
• Bilingual English/Spanish.

Schedule

• Per Diem / As Needed
• Flexible scheduling based on patient census and agency needs
• Weekday availability preferred; occasional weekend visits may be required

Compensation

• Competitive per-visit compensation
• Mileage reimbursement in accordance with agency policy
