Care Coordinator

Comfort Keepers group

Comfort Keepers of Central Illinois is seeking a leader to manage and drive continuing growth of our industry leading in-home care business. Must be an enthusiastic team player with a passion for working with people and performing meaningful work. CARE COORDINATOR — PART-TIME (PATH TO FULL-TIME) Comfort Keepers — CK Caregivers LLC — Peoria, IL 20–25 hrs/week to start — transitioning to full-time in early 2027 About the Role: Comfort Keepers of Peoria is growing, and we need a second Care Coordinator to join our team. You'll work alongside our Client Care Manager conducting in-home assessments, matching clients with caregivers, and making sure every new client's first weeks of service go smoothly. This role starts part-time (20–25 hours/week) and is expected to move to full-time in early 2027 as our client base grows — with a clear, guaranteed runway to more hours and more responsibility for the right person. This is a genuine growth position, not a temporary gap-fill. We are actively adding new clients across the greater Peoria/Bloomington area and need someone ready to grow with us. What You'll Do: Conduct in-home assessments with prospective clients and their families — understanding needs, explaining our services, and building the initial care plan. Match new clients with the right caregiver based on needs, personality, and schedule. Coordinate onboarding for new clients: paperwork, first-shift setup, and a follow-up check-in to make sure the fit is right. Manage day-to-day scheduling adjustments and respond to client and caregiver questions. Work closely with our intake systems and referral partners to keep the pipeline of new clients moving smoothly and promptly. Learn our full care coordination workflow, with the goal of taking on a full client caseload independently within your first few months. What We're Looking For: Warm, organized, and comfortable meeting new people in their homes — often during a stressful time for the family. Prior experience in home care, healthcare, senior services, or care coordination preferred; will also consider strong candidates from hospitality, healthcare admin, or client-facing backgrounds who bring the right temperament. Reliable transportation and willingness to travel within our service area. (Greater Peoria Area, with possible clients in Bloomington/Normal and Galesburg) Comfortable with basic scheduling software and phone-based coordination; we'll train you on our specific systems. Available for daytime weekday hours, with flexibility as the role grows. Participation in after hours on call rotation. Why This Role, Why Now? Growth with a clear timeline: We're expanding client volume now, and this position moves to full-time on a defined schedule — not an open-ended "maybe." Real mentorship: You’ll train directly alongside our current Care Manager, who brings years of hands‑on experience and will personally hand off her caseload and relationships to you over several months. A company built for the long haul: Comfort Keepers of Peoria has served central Illinois families for years, with a growing footprint that now includes patient transport, in‑home care, and expansion into new communities. Competitive pay: Based on experience; part-time hours now, full-time benefits eligibility as the role transitions. Comfort Keepers of Peoria is an equal opportunity employer. #J-18808-Ljbffr
